Norwalk Catholic is not able to dispense any medication to students unless we have a signed Medication Authorization Form on file. Below, you will find guidelines and required documentation links.

Administration of Prescription Medication
All medications must be sent in their original containers and labeled by a pharmacist. Parents must provide a container for each medication that includes the student's name, date, medication name, dosage, strength, and usage instructions. Please specify frequency, duration, mode of administration, physician's name, and the pharmacy's name and address. When filling the prescription, request a bottle specifically for school. If a bottle is needed for both home and school, the pharmacist can provide separate containers upon request. If there’s a change in dosage during the year, we cannot proceed until we receive a new Medication Authorization Form signed by the physician.

Administration of Non-Prescription Medication
Non-prescription and over-the-counter medications will only be administered if a completed and signed Medication Authorization Form is on file. Medications must be brought to school in their original containers from the manufacturer. Norwalk Catholic / St. Paul High School will strictly adhere to the dosing and frequency guidelines provided on the over-the-counter packaging. Any requests to change the recommended dose or frequency must be accompanied by a new Medication Authorization Form.
