Immunization Reminder for 2026-2027 School Year
All students in Kindergarten, Grade 7, and Grade 12 are required to have state-mandated immunizations completed prior to the start of school. If you choose to exempt your child from these immunizations, a valid exemption form must be submitted and kept on file. Click here to download the form. This must be completed by Friday, September 18, 2026.
All students in Grades K- 12 must meet immunization requirements or have an exemption form on file by Friday, September 18, 2026. Students who are not in compliance by this date will be excluded from school until proper documentation is provided.
Please submit proof of immunization or a signed exemption form to the school office no later than Friday, September 18, 2026.
