Please remember that Norwalk Catholic / St. Paul High School cannot administer prescription or over-the-counter medications to students without the appropriate signed authorization form on file. Prescription medications must be provided in their original, pharmacy-labeled containers, and any changes in dosage require a new Medication Authorization Form.
For over-the-counter medications, a completed OTC Medication Authorization Form must be submitted by a parent/guardian, and medications must remain in their original manufacturer’s packaging.
