On May 1st, Registration for all sports for the 2026-27 school year will be open on Finalforms. Once you have selected the sports your athletes wish to participate in, Finalforms will generate the necessary forms for you to complete or update. Registration will remain open until the start of each sports season.

The OHSAA Physical Examination Form signed by your doctor can either be uploaded to FinalForms to be reviewed or turned into Danny Grine in the Athletic Office.
All Athletes grades 5-12 are required to have a sports physical on file before the start of their sports season. If any forms are not completed prior to the official start of the sports season, athletes will not be able to participate until they are complete.
